If your test to scan a file does not trigger any actual scan, the first area to investigate is whether the anti-virus software is configured properly.
For example, see if the Real-time protection setting for Windows Defender is turned off. There may be some group policies that may disable the on access scan even after enabling the feature. Double check to make sure that Real-time protection is actually enabled.
